Our robust activity program offers comprehensive activities for memory-impaired residents at all ability levels. The program is staffed twelve hours each day, allowing residents to follow their own clock. At any given time during this twelve-hour period, at least two programs are offered to keep residents engaged, stimulated and to meet the varying levels of needs and interests.
A sampling of the offered activities include:
- Daily exercise programs, including Tai Chi, chair yoga and walking groups
- Sundowning programs
- Pet therapy visits
- Daily hair and nail grooming activities
- Tactile and cognitive stimulating activities
- Aromatherapy / hand massage
- Trivia and word games
Expressive Arts Therapy:
We use the creative arts as a form of therapy to return residents to a time and place they feel comfortable. The use of music, movement and art, whether used collectively or on their own, can reunite residents with parts of their former selves. The process of creation is emphasized rather than the final product. We use storytelling, dance, music, drama, poetry, movement and visual arts together to foster memories and positive emotions.
By offering intimate dining settings, residents experience greater opportunities for socialization. Continuity is provided by sharing meals with the same table-mates and receiving assistance by familiar staff. The overall dining experience is enhanced through the use of music, whether live or pre-recorded, and soft lighting in dining spaces.
We know family and loved ones are very important during this difficult transition. We work tirelessly to emphasize open communication between family members, residents and staff to ensure a comfortable environment. At German Centre for Extended Care, we offer support to residents and families needing assistance. Our support services provide education for families to help aid in understanding memory-impaired illnesses, dementia and Alzheimer’s care.
Our direct care staff partners with the residents and their families to meet residents’ daily needs for emotional, environmental and physical care while residing in our neighborhood. Care partners provide ongoing education and encourage family participation as part of the team approach. We offer monthly caregiver support groups, facilitated by a social worker and a member of the Alzheimer’s Association. Our library offers written and video materials to aid family members.
Residents are admitted to the Schrafft Pavilion Memory Support Neighborhood based on their requirements assessed in the pre-admission screening, which includes interventions required to meet care needs, functional status and the residents’ abilities to participate in the programs. The length of stay at the Memory Support Neighborhood is determined by the ability of the resident to benefit from the ongoing programs offered, as well as their health care needs.
The following medical services are provided at the Schrafft Pavilion Memory Support Neighborhood:
- Infusion therapy for the purposes of averting hospitalization, comfort and treating short term medical needs
- Wound care
- Pain management
- Nutritional management
- Respiratory management, including simple oxygen administration and inhaler therapy
- Diabetic management
- Cardiac care related to stable cardiac conditions
Residents admitted to the Schrafft Pavilion Memory Support Neighborhood could have multiple secondary diagnosis, such as:
- Renal failure
- Respiratory conditions
- Cardiac disease
- Neurological disorders
- Orthopedic disorders
- Neuromuscular disorders
- Endocrine disorders
Rehabilitation such as occupational, physical and speech therapies are provided on a short term basis to restore a resident to a baseline functional status after acute medical conditions. The role of rehab at the Schrafft Pavilion Memory Support Neighborhood is to develop and implement plans of care consistent with the habilitation philosophy of the Schrafft Pavilion.
