Carl J. Ludwig Transitional Care Unit
"The link between Hospital and Home"
For more than 85 years, the Altenheim has been recognized for the quality of
life provided to residents. Today, that same commitment has grown to
encompass rehabilitive and post-acute skilled services. The 20-bed Carl J.
Ludwig Transitional Care Unit offers spacious, comfortable rooms with
complimentary TV, telephone, electrics beds, and a private dinning area.
The Case Manager directs the plan of care from admission to discharge, working
closely with team members, as well as patient and family.
A rehab staff experienced in serving older clients incorporates the latest
technology while providing personal, individualized attention to each patient.
Clinical staff are fully conversant with the requirements of complex medical
issues, and all are employees of the German Centre not temporary
"pool" staff.
Visiting hours are unlimited, and the German Centre features plenty of free,
lighted parking. We are also on bus lines.
Outpatient therapies are available after discharge usually with th same
therapist.
The German Centre is approved for Federal Medicare, Harvard Pilgrim Healthcare,
Massachusetts Blue Cross, EverCare, and Tufts Health Plan.
We accept American Express, Visa and MasterCard.
You do have a choice on where you go. Be sure to make that known.
